[REDACTED] was walking into the dish room then he slipped and caught himself on the trashcan. He tried to pull himself backup with the trashcan but then he felt like his arm was dislocating and the pain from it made him let go of the trashcan and he fell all the way to the floor. [REDACTED] [REDACTED] walks over to to [REDACTED] calls a code red then helps him up to walk over to the kitchen office. [REDACTED] [REDACTED] walks into the kitchen office and talks to [REDACTED] about the incident. [REDACTED] [REDACTED] goes and gets an Ice pack for him and writes down the incident report and the witness statement. [REDACTED] asked [REDACTED] if he wanted to go get it checked out and [REDACTED] said he does not want to get checked out.
