PT WAS STARTING TO GET MORE AND MORE AGITATED AND REPEATEDLY TRIED TO GET OUT OF THE BED. THE DOCTOR ORDERED MEDICATION TO HELP THE PT GET MORE CALM. THE PT WOULD NOT HOLD STILL EVEN AFTER EDUCATING THE PT OF THE IMPORTANCE. THE WRITER THEN HELD THE PT DOWN TO HOLD HIM STILL AS THE NURSE ATTEMPTED TO INJECT THE MEDICATION IN THE SHOULDER. AS THE NEEDLE WENT IN THE PT MOVED AND THE NEEDLE FELL INTO THE WRITERS ARM.
