What happened
Butterfly tubing was caught on a piece of tape and pulled from patients arm. After removing the tourniquet from the patient I never engaged the safety capping on the needle. When I placed the needle in the sharps container it stuck to the sharps container. when I moved the needle to the inside circle of the sharps container it punctured my middle finger.
