Before the incident
Patient Care Pt in room 15 had an unwitnessed fall. [REDACTED] the same pt set the bed alarm off to go to bathroom and refused gait belt or walker. I was in the room waiting for him to finish so I could assist him back. Then I hear a bump and he is about to fall onto the floor. I proceed to catch him and have him use the walker back to bed. Then on [REDACTED] the nurse walked patient to bathroom and screamed for assistance because his knees buckled and he was not able to stand with walker. I ran in and proceeded to help to put the gait belt around patient and help him stand with a few other staff members to get him safely back in bed. My hand has been hurting due to these prior incidents.
