What happened
While conducting base duties employee [REDACTED] wanted to file an incident that occurred prior to her shift. Employee stated to me while crossing the street from the Waldorf Astoria she slipped and fell hitting her head left side of her head and left side of the knee causing her discomfort during her shift. Employee stated to me during her shift she was experiencing mild headaches and possibly bruising around her left knee and left side of her head. First aid was offered but employee refused and will update us on her condition once she evaluates her conditions. Employee also declined 911 emergency assistants.
