What happened
EE had given the patient a SQ injection of heparin in back of patient 's arm. She had placed patient 's gown over shoulder to have clear view of injection site. As nurse went to withdraw needled gown fell down over syringe and entangled syringe as she attempted to pull needle out of patient 's skin. After syringe untangled from patient 's gown EE went to activate safety device and noted there was blood in glove near left thumb. She left source patient 's bedside and went to sink in patient 's room removed gloves and attempted to wash hands.
