Before the incident
Employee was being trained on safe operation of the Walkie[REDACTED] (PIT). He was in the process of demonstrating his ability to operate the Walkie[REDACTED] (PIT) during the practical portion of the training class (i.e. practicing maneuvers) on the Walkie[REDACTED] (PIT).
What happened
The employee was practicing maneuvers on the [REDACTED] ([REDACTED]) during the practical demonstration part of the [REDACTED] training when he lost control (lost grip of the lever) and fell over the control arm after the [REDACTED] came to a sudden full stop thus landing landing on left shoulder. The [REDACTED] stopped due to the lever being released (let-go) on a very sharp left-turn and the lever suddenly going into the full horizontal (i.e. brake stop) position thus the momentum causing the employee to lose balance and fall over the lever and onto the floor landing on his left shoulder.

The employee suddenly fell off the Walkie-Rider onto the hard concrete floor when the PIT came to a sudden stop. The Walkie-Rider came to a sudden stop due to employee losing grip of the control lever while making a very sharp left turn. The momentum caused him to lose balance and fall off onto concrete floor landing on shoulder injuring it.
