What happened
On [REDACTED] at approximately [REDACTED] an Inventory Control associate fell from a front rider electric pallet jack while operating it near the back of the warehouse in the vicinity of aisles 37 through [REDACTED]. According to video footage and witness accounts the associate was traveling on the pallet jack when it came to a sudden stop causing the associate to lose balance and fall landing on their back side. A nearby associate witnessed the incident and assisted the fallen associate in getting back up. The injured associate resumed use of the pallet jack and did not report the incident to any member of leadership at the time. Footage from camera DVR2 (Aisle [REDACTED] Middle) timestamped at [REDACTED] captures the incident and is attached to this report. Additionally a statement from the witnessing associate is included.
