What happened

On [REDACTED] at approximately [REDACTED] an outbound supervisor was operating a CROWN center rider pallet jack to transport a stack of pallets from the cold side to the dry side while concluding end-of-shift cleanup and organization duties for the outbound team. The stack of pallets being transported was unevenly stacked and unstable. As the supervisor traveled through an intersection on the dry side of the warehouse the unsecured stack shifted forward and fell striking the supervisor on the head left forearm and left hand. Following the incident the supervisor reported pain in the affected areas and immediately received onsite first aid treatment. The supervisor will be meeting with the onsite Fit For Work representative for additional evaluation and onsite care. At this time the supervisor has declined outside medical treatment with the company doctor.

Contusion to left arm contusion to left hand and nondisplaced fracture to left ulna styloid process.
