What happened
On [REDACTED] at [REDACTED] an operations manager was traveling up aisle 42 on a CROWN end-rider pallet jack to complete a slot check. After finishing the slot check the manager began reversing out of the aisle forks-first to turn the pallet jack and head back down the main travel path. While reversing and turning the pallet jack swung outward causing the manager’s left hip leg to strike the metal racking which led to the manager falling after the equipment came to a stop. As a result the manager is experiencing pain in the left hip leg and will be seeking outside medical evaluation at the company doctor. The CROWN end-rider pallet jack was removed from operations and will be evaluated by the CROWN mechanic team for any functional issues.
