Before the incident
Employee bent over to retrieve a bottle of dawn detergent off of the chemical shelf in the dish room. When he reached down to grab it he knocked a bottle of delimer off of the shelf. The bottle of delimer was used previously by someone else. The foil cap between the bottle and the plastic cap was toern and the plastic lid of the bottle was not screwed on to the bottle tightly. When the bottle of delimer hit the floor the lid came off and the delimer splashed upwards and got into Mo 's eyes.
What happened
Employee States: Employee stated that he went to grab a bottle of [REDACTED] a bottle of [REDACTED] slipped. The [REDACTED] bottle was not capped properly and part of the seal was open and some splashed of the [REDACTED] got into both of his eyes.
