moving empty bread racks onto salvage truck to return. wheel base was stacked on top of the empty racks to return on salvage truck. team member was pulling racks onto ramp and wheel was stuck betwwen floor and ramp racks shifted and wheel base fell off top onto team member
What happened
Employee States: I had started up the ramp to the salvage truck with the red plastic rack and the wheels stopped short. The extra wheel base placed on top of the rack fell down on my head.
