Sprain of right shoulder from trip and fall [REDACTED] tripped over forklift forks while wrapping a pallet in the aisle. [REDACTED] reports that forks were lowered to the ground and pallet was approx 2.5 to 3.5 feet away. [REDACTED] stated he felt dizzy during wrapping and may have caused him to lose balance and tripped over forks nearby. [REDACTED] tripped over a fork and fell on front right shoulder. [REDACTED] reported incident to EHS and was treated with an ice pack in triage and [REDACTED] requested going to the clinic because shoulder movement was severely limited.
