Before the incident
the employee was working on level 5 connecting steel alongside another ironworker. They had recently completed installing the final piece of the three-piece tree assembly.
What happened
the team had completed installation of a three-piece tree assembly on the 5th floor. At this time the journeyman ironworker detached the rigging and attempted to secure it to his secondary self-retracting lifeline to reposition the cable on the opposite side of the beam. During this maneuver the cable slipped from his hand causing employee to lose balance. Initially the employee maintained a seated position on the iron however the sudden force of the rigging falling caused him to lose balance and be pulled into a suspended position. He fell approximately 9 feet.
