Before the incident
Spoke to employee and she said that she was the scrub nurse in the OR and was in front of a mayo stand that was set at her chest level that the 10-blade scalpel was on that she handed to the surgeon. The surgeon made the first incision and the 10 blade was put back on the mayo stand. While the employee was reaching over it the 10-blade scalpel got caught in the paper sleeve of her gown and poked her right forearm. She immediately said that she was poked and saw blood on her arm. The mayo stand was covered with [REDACTED] and the scalpel was removed so no risk cross contamination to the patient. The employee left the OR and washed her arm with soap and water reported the injury to her manager and completed a spark report. She was seen by Concentra on [REDACTED] the date of the injury. Employee is scheduled for follow up at Concentra in 2 weeks. Source consent was signed by the patient 's mother which is not acceptable as there is no documentation supporting this being legal.
