EE was assisting a patient with toileting using a belt. The patient was very unstable; as patient attempted to sit on the toilet patient began to tip and inadvertently bumped into EE causing her to be pushed into the shower area and hit the back of her head.

EE states EE was sitting on a stool in the kitchen at work and her feet got tangled and EE fell and hit the back of her head and hit her back and EE went home [REDACTED] EE woke up dizzy and fell back hitting her left side ribs.
