Transporting a client to their medical appointment from the Agency us while they were sitting on their walker with wheels.
What happened
The walker got caught on a cement slab and the client fell back. Employee attempted to stop the client and in doing so they lost their balance and stumbled forward however they did not completely fall to the ground.
