Before the incident
Employee was forming a part with the press brake and it was a 2-step process. He was on the second step. He was holding the part and forgot that the metal would retract toward the tooling and he pinned his right middle finger in the machine.
What happened
He was forming a part and it needed to be double bent. When he was doing the second bend he decided to hold it. He forgot that the metal would bend towards the tooling and it pinned the finger causing the tuft of his middle finger to fracture and also cut it.
