Before the incident
Employee was untying a trim bag in extrusion.
What happened
When the trim bag was released from the trim pipe the other operator used an electric pallet jack to pick up the trim bag that was sitting on top of a small pallet. Unknowing to the other operator the forks extended past the pallet and were under the 4ft.platform ladder the injured operator was standing on. When the other operator raised the forks they lifted the platform ladder and caused the injured operator to lose his balance and fall to the floor landing on his right shoulder.
