What happened
Due to the web break the slitter became unwebbed meaning there was no film in the machine and the slitter stopped. The limit switches on the dancer rollers prevent the machine from running or being started.
Injury or illness
When the employee began re-webbing the plastic film through the slitter free spinning idler rollers were still spinning at an elevated speed of rotation and the employee did not realize this. The elevated rotation of the idler rollers pulled the employees hand in between and caused a crush & abrasion to the left hand.
