What happened
On [REDACTED] at approximately [REDACTED] I was in the STOCKROOM when I witnessed an incident involving [REDACTED]. She was walking in the area when she tripped over a box that was in the path. Upon tripping [REDACTED] fell to the ground immediately. I could see that she was in a great deal of pain and she visibly grimaced after hitting the floor. She remained on the ground for a moment unable to move. After a brief pause [REDACTED] attempted to get up but as she did she was in too much pain. as I attempted to help her up I heard multiple loud cracks coming from her.
