IE was tasked to be lifted to top of Sub-[REDACTED] welder (approx. 12ft) via forklift and man basket to install some rigging on a lifting point at the top of the machine to adjust position. As IE was being lowered by forklift the decent of the mast suddenly stopped while the lowering chains were still moving and becoming slacked. [REDACTED] the mast became dislodged and suddenly dropped with a free fall approx. 1.5ft. The fall caused a volent shake of the man basket and forklift causing IE to strike his mouth on the guard rail of the man basket and fall to the floor of the platform. IE soon arose from the fall dusting himself off and evaluating his injuries. He started to bleed heavily and his surrounding peers escorted him to the nearest break room.
