On [REDACTED] at [REDACTED] an incident occurred in the warehouse An operator (DOH: 62122) stated they were picking product (6 cartons of POLYFILM weighing 58 bx) in the JP aisle and felt a pain in the lower back They immediately informed the Unit Manager and Axiom was contacted at [REDACTED] The pain level was described as a level of 5 and first aid recommendations were given The operator has been crosstrained and was given a project to allow positive improvement of the low back Axiom was unable to reach the employee on [REDACTED] but was able to make contact on [REDACTED] where the operator reported a pain level of 4 but improving Multiple conversations and daily follow ups were had with the operator and other members of leadership throughout the [REDACTED] Axiom was able to follow up on [REDACTED] and the pain level was 36 (moderate pain with range of motion) On [REDACTED] during a follow up call with Axiom the employee requested medical evaluation given the lack of improvement in t
