On [REDACTED] at approximately [REDACTED] a fulltime employee with five months of experience was operating an EPJ in the Temp Dock area when a collision occurred with a Dock Stocker The EPJ operator was making a Uturn to begin a new task while the Dock Stocker operator was traveling through the area toward the wrapper Miscommunication about their intended paths resulted in both PITs colliding at the platform area of their equipment The collision occurred at a low speed Following the incident the EPJ operator reported right hip pain to their UM First aid was promptly administered and the Axiom service was notified No further immediate incidents were reported On [REDACTED] the EE reported for their shift but later decided they were unable to perform their assigned duties and went home resulting in a lost time recordable On [REDACTED] the EE underwent evaluation at the Occ Med clinic and was diagnosed with right hip pain They were issued work restrictions that the site could not accommoda
