Before the incident
Employee was riding on the drivers side step while backing up to service 4yd container.
What happened
Helper [REDACTED] Clodfelter was positioned on the rear driver 's helper step during the backing maneuver. As the vehicle reversed [REDACTED] observed some foliage and in an attempt to avoid being dragged through it leaped from the rear step. Unaware that the area he jumped into was elevated the combination of tall grass and damp conditions caused him to slip and fall. At this point [REDACTED] noticed in the driver 's side mirror that [REDACTED] had fallen. He immediately applied the brakes and set the parking brake. Despite these actions the rear tire of the vehicle came to rest on [REDACTED] 's right foot.
