Before the incident
Normal Duties. Walking to Pad Area
What happened
Associate Employee (EE) and coworker was going out to pad area. The primary path taken to this area in order to avoid other hazardous areas (i.e. Boiler Room and Vulcanizing Oven) is through this full vertical lift overhead door. There is not a personnel door in this area. As the EE was walking out the coworker went ahead the EE stopped and watched the door raise to height. At this moment the drive chain failed and the door came down striking the EE on the left shoulder blade and on both knees. As the EE was attempting to turn out of the way the pressure of the door caused EE to have a left ankle inversion and fell to the ground onto buttock and then on to back (this was all on the same floor level).
