Before the incident
Operator was operating a standup forklift He just got done taking pallets outside and was retuning back into the warehouse when the top of the mass hit the above entrance way when entering the building operator failed to lower his fork after dropping of pallets He instead started to drive off before full lowing coming back into the building and striking the building This caused the forklift to then slingshot operator out of the equipment and on to the floor of the warehouse
