What happened
The EE was struck by a channel cart at [REDACTED] as it was lowering to floor after dumping a load of oil into the [REDACTED] hopper. The channel cart slipped out of the locking guide slots and the force of the strike pushed the EE backwards and rotated him face down towards the floor by the control area. The EE was taken from the production floor to the first-aid room where he complained of pain to his right side and trouble breathing. The EHS Manager had the Office Administrator call [REDACTED]. [REDACTED] FD arrived at [REDACTED] and examined the EE. They took him to Good Samaritan Hospital in [REDACTED] at [REDACTED].
