The employee was covering a coworker on the Spring Mix lines and after arranging two product bins was moving forward to pick up an empty bin.

The employee was covering for a coworker on the Spring Mix lines and was arranging two product bins. After arranging the bins the employee moved forward on another forklift to pick up an empty bin. While reversing the forklift without looking back the employee was struck on the rear by another forklift. Upon investigation and review of video footage it was observed that neither the employee nor the coworker looked back while reversing. Both operators failed to take proper precautions when their forklifts counterweights came into contact.
