Before the incident
The employee exited Room 2 in reverse with six crates of carrot baggies.
What happened
The employee was exiting Room 2 in reverse while transporting six crates of carrot baggies. At the same time another forklift operator reversed and positioned the forklift forward to pick up empty crates. When the employee noticed the other forklift the second operator began reversing at high speed resulting in a collision between the counterweights of both forklifts. After reviewing camera footage it was observed that the employee did not maintain sufficient space between the forklifts while operating contributing to the collision.
