What happened

[REDACTED] had completed bending 8 inch x 4 inch for 10 inch round doors on the 10 ft. press brake in the Access Door Department. [REDACTED] then began the die changeover to 12 inch x 8 inch for 14 inch round doors. During this process [REDACTED] arrived to assist. [REDACTED] observed that the idle 5.5 die on the left side of the press brake interfered with the new 12 x 8 for 14 inch setup. In an attempt to resolve the issue [REDACTED] proceeded to remove the idle die without first lowering the top die to the proper height required for safe removal. To facilitate removal [REDACTED] picked up two 2 x 4 wood blocks from the drill press and positioned on top of the bottom die left side and loosened the top die screws. Once loosened [REDACTED] pushed the top die resting on the wood blocks toward the left using both hands. At this point one of the wood blocks slipped out of position causing the top die to roll off the press brake. As the die shifted it struck [REDACTED]s right hand pinching
