EE lb1 stated on [REDACTED] that his been feeling soreness on his lower back for a couple of days. He was placed on modify duty. 1001 at [REDACTED] the Triage Nurse was contacted and she communicated that she was going to contact him directly. 1002 EE 1 was set up with an appointment by the triage Nurse. 103 - at [REDACTED] EE meet with the EHandS I asked how he was feeling. EE stated better but is still experiencing some discomfort on his right side of his back and sometimes it comes to the front. EE is taking over the counter ibuprofen for the pain. 1003 EE is on his way to the clinic that was referred to him by the Triage nurse with his manager. [REDACTED] Returned to work form visiting City MD. EE was diagnosed with strain of lumbar region. Strained of muscle EE May return to work with the following temporary restriction. No lifting of bending twisting or kneeling. For 3 days Effective on [REDACTED] - was not prescribed any medication. [REDACTED] had a follow with city md and was c
