What happened
EE was operating the right side of the 3 inch 8 inch reducer machine to produce 5 inchx4 inch reducers. While operating the machine in manual mode the EE inadvertently pressed the inchclose die inch button on the HMI causing the die to close and pinch the tip of the left index finger.
