On [REDACTED] the officer was called by HR [REDACTED] to assist with the team member who is stating that while working in the dept. 3pT lifting a chair he felt a pull in his back from lifting the chair and stated that this incident happened on [REDACTED] and wasn 't reported. [REDACTED] was up by HR and [REDACTED] who is the HR Manager asked [REDACTED] if he was doing okay and stated to [REDACTED] that his back hurt from [REDACTED] lifting that he asked for help from the supervisor and others but no one helped him and he continued to just work. The Officer called the Medcor nurse. [REDACTED] recommended that [REDACTED] see a provider andLogan was sent to Concentra for medical attention.
