The report # 1050 is reference to incident that occurred with a team member on [REDACTED]. On [REDACTED] the team members is stating that he is still in pain from the fall incident that happened on [REDACTED] [REDACTED] security officer called Axiom Medical for assistance with the team member. Axiom nurse recommended the team member to take some Tylenol and use heating pad or ice but the team member states he did this procedure the [REDACTED]. Team Member statement: On [REDACTED] He was coming down with the pallet jack from the ramp (where house) when the pallet jack twisted and pull him down and fall. He hurt his left shoulder and finger too. This is supposed to be statement # 1050 however there is no previous record.
