On [REDACTED] she visited an urgent care on [REDACTED] because she thought her blood pressure was elevated and the provider stated she has a concussion which she states is related to the incident with the resident. She couldn 't provide the full name of the patient during our conversation but stated his last name is [REDACTED] first name initial is C. She was given an incident report at the end of our meeting and asked to complete it. She took the form and said she would slide it under my door so I would have it when I returned to my office in the morning. During our conversation she did not mention she had trauma to her head of any kind.
