Before the incident
employee was diagnosing a weld machine that was not cooling properly. Looking at the coolant hoses and the coolant tank for blockages.
What happened
Employee was using a pliers to grip the coolant hose to remove from the coolant tank. Employee was holding pliers in his right hand wile griping the hose and turning it back and forth from sided to side to get the hose to come free. Employee had about 2-3 twists when he felt extreme pain in his right bicep shoulder.
Injury or illness
employee stated he was not able to be driven to the ER due to the pain and having to hold his right arm with left arm. EMT 's were called and employee was taken to hospital with believed bicep shoulder strain. Employee ended up having a broken RT arm with unknown underlying health issues to that arm.
Object or substance involved
gripping the pliers and using the turning motion to get the coolant hose free from the tank.
