What happened
employee was using the jib crane to raise up a part so it could be formed in the machine. The machine was then activated to clamp down on the part being formed and the employee raised the part too high with the jib crane remote. This caused the plate clamp to be pulled off the top of part being formed and the part fell back as it had no support while being clamped in the machine.
