Before the incident
A pallet had jammed on the conveyor going into the freezer and the primary wrapper film was broken. EE had just fixed the pallet before she tripped.
What happened
EE was unjamming a pallet on the last turn table before the freezer when she noticed that the primary wrapper film had broken. EE walked over the conveyor at the cross over stairs. When she stepped off of the last step she felt her foot get caught in the tread of the step causing her to fall to the floor landing on her left knee and rolling on her side.
