Before the incident
EE was trouble shooting [REDACTED] 5. Cases fell off from pallet on hoist. EE pushed them down and pulled the using a long metal rod that is also used to pull out empty pallets.
What happened
EE stated she pulled a muscle while pulling fallen cases from the hoist. She was working in Alvey 5 the motor was failing and caused cases to fall. She had to bend down under the platform and pull cases out using a metal rod. EE refused treatment and returned to work. She did see FFW onsite.
