Before the incident
Employee had just got back from break when he saw his coworker had a frozen pot on the platform.
What happened
Employee used hoist with hooks to lift the full tundish pot from the platform. when he lifted it and started to move away the pot tip and spilled metal onto the wet platform. this caused a the metal to pop and throw metal in the air.
