Before the incident
Employe was inserting a pallet into the ice injector with a forklift.
What happened
Employee was driving a forklift and was going to insert a pallet into the ice injector when he noticed a loose piece of wood about to chip off and fall into a vat of water under where the employee was going to place the pallet. The employee reached in between the forklifts mast with his right arm and under the pallet that was slightly raised above a flat platform. He rotated his body right to reach deeper under the pallet doing this caused his body to flick on of the levers lowering the pallet on top of his hand.
