Before the incident

Employee was working on re-assembling hook rollers on the Shiploader chute rotate platform. He was using a sledgehammer to knock the assembly in place so it could be bolted up.

What happened

While working on the Shiploader hook rollers the employee had placed his unused bull pins in open bolt holes to keep them from falling to the surface below. The employee forgot about the placement of the bull pins and was attempting to hit the roller assembly with a sledgehammer to line up the bolt holes so the bolt could be inserted. The bull pin was unknowingly in the path of his hammer swing (tapered end of the bull pin was down in the swing radius). When the employee swung the hammer his hand came in contact with the small or tapered end of the bull pin. This caused a gash in the webbing of his hand between the thumb and index finger on his right hand. The employee was not wearing proper PPE. He had on nitrile gloves instead of the required leather gloves for that job.
