EE was attempting to line up a set of disc gangs with disc frame to insert pin. Gangs were suspended from overhead crane employee was applying pressure with a pry bar.
What happened
EE inserted right-hand index finger into the area that was bound-up not allowing the pin to be inserted. When the bound-up area released the EE 's finger-tip was amputated.
