The team member bypassed a safety protocol. The last wheel on the machine had the pneumatic arm stuck under it. They repeatedly tried to cycle the machine building pressure in the line then they manually bypassed the safety device and moved the wheel by hand and the stuck arm broke free and shot upwards hitting the team member in the face.
