What happened
Employee #1 was holding the release lever open on the hopper while Employee #2 was dumping the hopper in the dumpster. The hopper wouldn 't dump so employee #3came with another forklift and tried pushing the hopper while employee #1 remained holding the lever and standing under the forks. One of the forks slipped causing it to dislodge from the carriage and fall on Employee #1 's left shoulder and torso.
