Before the incident
Laceration to the head unconscious While performing the main chain change on the Sidel pasteurizer the crew had the chain pulling bar linked to the old chain via a steel pin The pulling bar was rigged to a forklift with a rigging nylon strap The chain was going to take the path out of the machine and through the middle handrail of the catwalk While 2 coworkers observed the discharge for snags 2 coworkers helped guide the rigging bar and strap as the forklift driver began to put tension on the rigging assembly to remove the old chain As the slack was taken out the 2 coworkers guiding the strap were free to evacuate from being in the line of fire The chain began to move towards the discharge and approached the catwalk handrail Just as the team was communicating to back away from the area and be free of the line of fire the eyelets which engaged the steel pin from the pulling bar to the chain broke sending the pulling bar towards the forklift striking one of the co workers in the line of
