What happened
Employee did not install engineering controls to prevent trailer from being moved away from the dock per MAPF policy. Employee failed to remove engineering controls from the completed trailer at door [REDACTED] and install engineering controls at door 21. A yard driver assigned to pick up the completed trailer at door [REDACTED] saw that the only trailer available to be transported was at door 21. Yard driver hooked to the trailer and pulled the trailer slightly away from the dock door. Employee then drove into the trailer and forklift became stuck.
