Before the incident
On [REDACTED] on or about [REDACTED] I received a phone call to let me know that a team member in the [REDACTED] had a particle in his eye and needed assistance Our team member was flushed his left eye and when he was not successful in clearing the particle from his eye other team members assisted him with eye was solution from the first aid cabinet When I arrived I also tried to assist in the removal of the particle from his eye with no success at which time I realized that our team member required the assistance of a medical provider to remove the particle and I drove him to Concentra Urgent Care in [REDACTED] Rock where the treating doctor was able to remove the particle and stated that it appeared as a clear filamentous sliver from the eye Our team member shared that he was cutting the stretch wrap from three pallets in front of shipping door number ten and when began to cut the stretch wrap off of the third pallet is when we felt a particle in his left eye Our team member who is a
