Before the incident

What happened
The hoist was connected to the eyebolts of the mold. He was standing between the wheel removing vise grips. One of the hooks was not properly seated and the chain was not taunt. Causing the weight of the mold to not be properly balanced or supported by the hoist. His right hand was around the wheel while removing the grips with left hand. He removed the last grip. The hoist became the bearer of the weight causing the improper hook to disconnect from the eyebolt. This caused the frame of the mold to slide trapping his fingers between the mold and the wheel.
